General summary
Damasio's central claim reverses a long tradition that opposes reason to emotion. Drawing on neurological patients whose ventromedial prefrontal cortices are damaged — the nineteenth-century railway foreman Phineas Gage, and his modern counterpart "Elliot" — he argues that when emotion and feeling are stripped away, reasoning and real-life decision-making collapse, even though intelligence, memory, language, and attention remain intact. Part I lays out the clinical evidence: a specific brain region, when destroyed, consistently yields the paired impairment of decision-making and emotion. Part II builds the theoretical scaffolding — organism, body, brain, mind, images, dispositional representations, biological regulation, and a two-tier account of emotions (primary/innate and secondary/acquired) and feelings (the perception of body states) — and culminates in the somatic-marker hypothesis: emotion-linked body signals, overt or covert, mark options and outcomes, pre-selecting and biasing choices before deliberation. The 1848 accident: a tamping iron blows through Gage's skull, and he survives physically intact but morally and socially transformed. Damasio reconstructs the case from Harlow's and Bigelow's contemporary reports, uses it to introduce the collision of the localizationist and holist camps of brain science, and (via an aside on phrenology and Gall) argues for specialized brain systems without "centers." Gage's real lesson, ignored in his day, was that social conduct and planning depend on specific brain systems.

Chapter 2 — Gage's Brain Revealed
Because no autopsy was done, Hanna Damasio and colleagues reconstructed Gage's likely lesion from his preserved skull using neuroimaging (Brainvox) and computer simulation of the iron's trajectory. An extended aside lays out basic neuroanatomy — gray and white matter, cortex and nuclei, neurons and synapses, the levels-of-architecture hierarchy. The conclusion: the iron spared motor and language cortices but destroyed the ventromedial prefrontal region.

Chapter 8 — The Somatic-Marker Hypothesis
The book's titular thesis. Faced with complex personal/social choices, pure "high-reason" cost-benefit calculation is too slow and overloads memory. Somatic markers — feelings arising from secondary emotions, attached by learning to predicted outcomes — bias options in advance: negative markers as alarm bells, positive ones as incentives. They do not decide for us but pre-select, and can operate covertly (the basis of intuition) via an "as-if" loop. The ventromedial prefrontal cortices are the key acquisition site. Emotion can also mislead (the flying-versus-driving bias), but its absence is worse.

Chapter 9 — Testing the Somatic-Marker Hypothesis
Two empirical lines. (1) Skin-conductance: prefrontal patients viewing emotionally charged images fail to generate the autonomic responses normals show, though they fully grasp the content — measurable proof of "to know but not to feel." (2) The Gambling Task: normals learn to avoid the high-penalty decks and generate anticipatory skin-conductance responses before choosing badly; ventromedial-frontal patients keep choosing the bad decks and show no anticipatory signal — a "myopia for the future."

Damasio argues the body is not mere life-support but a content of mind, providing its ground reference and metric. Perception is active and body-engaged (you feel you see with your eyes). He rejects the "brain in a vat" as a normal mind, and sketches a neural theory of the self as a continuously reconstructed biological state — built from autobiographical memory plus primordial body representations — and of subjectivity as a "third-party" image of the organism perceiving an object.

Chapter 11 — A Passion for Reasoning
The synthesis and the naming of the error. Reason is constructed on a bodily "drive," a passion for reason, and depends on systems that also process feelings. Sociocultural implications (education, violence, over-medicalizing feelings) are drawn out. Descartes' error is the abyssal separation of mind from body — of the most refined operations of mind from the biological organism — encapsulated in "Cogito ergo sum." Damasio counters: being precedes thinking.

The "human heart in conflict" enlists neurobiology in human affairs; Western medicine's Cartesian split neglects the mind and the mind-body loop; a candid note on the limits of current neurobiology (sheer complexity, individual and cultural variation); and "Leverage for Survival," where pain and pleasure are the levers of decision-making, suffering is distinguished from the sensory image of pain (the leucotomy patient whose pain remained but whose suffering ended), and homeostasis is proposed as the root of human values.

What the book does NOT say
It does not claim emotion should replace or override reason, or that "following your heart" suffices — Damasio explicitly rejects this reading (see the 2005 Preface), arguing emotion assists and holds a dialogue with reason.
It does not claim somatic markers necessarily operate nonconsciously, nor that skin-conductance responses are somatic markers (they are indices of them) — the Preface corrects both misreadings.
It does not reduce ethics, altruism, love, or free will to neurobiology; Damasio repeatedly insists that partial biological explanation does not debase or eliminate these, and that humans retain some freedom from biological and cultural constraint.
It does not present its central claims as settled fact: the somatic-marker hypothesis and the neural theory of the self are offered as working hypotheses open to revision.
